Required Forms for New Students

In addition to the forms listed below, New York State Education Law requires that students receive vision, hearing and scoliosis screenings as part of the school health services provided by each public school district. The purpose of these screenings is to detect the presence of problems likely to impede a student’s learning. If the results of the screening indicate that your child requires further follow-up, you will receive written notification.

New Students Are Required to Submit:

2023-2024 New Student Annual Health History Form
Health Exam Form
Dental Health Form
Vaccination Records (request a print out from your child's doctor)
